Getting this right for Issue 6 is really important.

We have done a few audits already against Issue 6 and the root cause action plans we are seeing for non conformities are not very good so far.

Remember that by doing root cause analysis you are looking to prevent recurrence of the non conformity. You need to do this process properly using one of the many techniques available (check the internet) so that you can give acceptable plans that we can then verify at the next audit. If you are stuck on this try the 5 Why's technique which is possibly the simplest. We need to see that you have considered the basic aspects that have led to the issue.
